MindWeal Care Transition and Discharge Guidelines

Our goal at MindWeal is to provide continuous, comprehensive mental health care. However, there are instances when we may need to transition a patient out of our service. Below are the circumstances under which a discharge could occur, and how we handle these situations with your care in mind:

Appointment Compliance

Two Consecutive Missed Appointments

Failing to attend two scheduled appointments in a row, either by not showing up or by canceling less than 24 hours before the scheduled time (excluding weekends and holidays), may begin the discharge process.

Three Missed Appointments Within a Year

Missing three appointments within a year, whether by not showing up or canceling late, indicates repeated noncompliance and may lead to discharge.

Treatment Compliance

In cases where a patient remains non-compliant with their treatment despite our dedicated efforts, it may indicate that a more intensive or higher level of care or a different setting is necessary for their well-being. Under such circumstances, we may need to consider discharge as a course of action. Rest assured, we will provide appropriate referrals to ensure continuity of care and support the patient's health journey.

Transitioning to Adult Psychiatry Services

As our patients reach the age of 18, we carefully review their treatment plans to confirm that MindWeal can continue to meet their evolving needs. Should their journey require adult-specific services, such as certain psychotropic medications, depot injections, Transcranial Magnetic Stimulation (TMS), or ketamine treatments, which we do not offer, our commitment shifts to helping them connect with the appropriate adult service providers. Our goal is to ensure a smooth transition, with every patient receiving the right level of care as they step into adulthood.

Throughout the discharge process, our focus is on ensuring a responsible and supportive transition. We are committed to providing the necessary resources and referrals to maintain the continuum of care and support the patient's mental health journey.
