Fee Schedule For Out-Of-Pocket Services

At MindWeal, we are committed to providing clear and transparent billing practices for our patients. Please review our fee schedule for out-of-pocket services below.

Credit/Debit Card on File: 

To facilitate your care, we require an active credit/debit card on file. Your card details will be securely stored and managed through Stripe.

Insurance accepted visits: 

We accept the following insurance plans:

• BCBS of Illinois (commercial plans)
• BCBS PPO plans of most states
• United Healthcare (commercial plans)
• Cigna (commercial plans)
• Health Alliance Network
• Anthem (excluding HMO plans)
• Out-of-network for TriCare

Your final charges will depend on the maximum allowed charges as per our contractual rate with the insurance company. Generally, you are responsible for your copays and coinsurance, but additional charges may apply if your deductible has not been met.

Out-of-Pocket Clinical Visits: 

If we do not accept your insurance, you will pay out-of-pocket for your visits. Payment for self-pay appointments is due in full 48 hours in advance of your scheduled visit (excluding weekends and holidays).

Initial Consultation

Billed at a flat rate, which includes the entire new patient evaluation without any additional charges. Check your preferred location for the exact rate below.

Follow-Up Visits

Standard follow-up visits are charged at the base rate for a Level 2 visit, which includes 21-30 minutes face-to-face with our Provider. If the visit is completed in 20 minutes or less, you will be charged for a Level 1 visit and refunded the difference. If the visit extends beyond 30 minutes, the additional cost for a Level 3 visit will be charged to your card on file after the visit. Check your preferred location for the exact rate below.

Out-of-Pocket Charges for Advanced Testing

Included Services:

• In-office or remote computerized testing.
• Comprehensive review of all records by Dr. Bansal, encompassing the testing performance, initial evaluation, prior clinical notes, historical tests, and questionnaires from educators and caregivers. It also includes discussion with your clinical provider at MindWeal.
• A detailed report with findings, diagnoses, medication suggestions, additional testing if needed, school accommodation requests, and guidance for teachers and parents.

• Initial comprehensive psychiatric evaluation fee.
• Post-testing consultations to refine the medication management plan 
• Charges for the executive functioning training program.

Other Services

For any additional services not covered by your insurance, we will only charge your credit card on the day the service is provided, and only after we have clearly communicated the charges to you and received your explicit consent.

Clinical Consultation with the Psychiatrist 

New Patient Evaluations are always conducted by Board Certified Mental Health Nurse Practitioners, in collaboration with Dr. Bansal. If your provider refers you to Dr. Bansal for a one-time consultation, a 30-35 minute session is offered at $200. This fee includes Dr. Bansal's pre-consultation review of your records and coordination with your provider. Should further sessions with Dr. Bansal be needed, they will be billed as follows:
