      Document Processing (Paperwork) Policy

      Thank you for choosing MindWeal for your child's mental health services. We are committed to transparency and clear communication regarding our fees. Below are the guidelines concerning charges for paperwork completed by MindWeal providers and staff.

      Charges for Paperwork

      Treatment-Related Documentation:

      There are no additional charges for letters written as part of a recommended treatment plan.

      Other Paperwork:

      • Short Forms (5-10 minutes): $25
      • Extended Documents (15 minutes or more):
        • $50 per 15 minutes if completed by a Nurse Practitioner (NP)
        • $75 per 15 minutes if completed by a Psychiatrist
        Upon your approval of the fees, charges will be added to your account balance or your credit card on file will be charged.

      Processing Time 

      Please allow us a period of 5 working days to complete the requested form. This time frame ensures that we can accurately and thoroughly complete your paperwork.

      Clinical Visit Requirement

      A clinical visit is required for paperwork if the patient has not visited the clinic within the last 30 days.  This is to ensure that the most current and accurate information is provided.

      Amendments and Corrections

      • Paperwork will only be amended in the case of clerical errors.
      • If paperwork needs to be redone for reasons other than our clerical errors, an additional charge, as outlined in our fee schedule above, will apply.
