# Document Processing (Paperwork) Policy

Thank you for choosing MindWeal for your child’s mental health services. We are committed to transparency and clear communication regarding our fees. The following policy applies to paperwork completed by MindWeal providers and staff.

### What Is Not Charged?

There is no additional charge for letters or forms that are recommended or initiated by a MindWeal provider as part of the patient’s routine treatment plan.

Examples include:

- School accommodation letters recommended as part of the treatment plan
- School medication administration forms
- Prior authorization forms for insurance companies
- Letters to insurance companies supporting treatment recommended by a MindWeal provider
- Other treatment-related letters or forms recommended by the provider as part of ongoing clinical care

### What Is Charged?

Paperwork requested by the patient or family that falls outside the patient’s routine treatment plan is generally considered a separate billable service.

Examples include:

- Family and Medical Leave Act (FMLA) paperwork
- Disability paperwork
- Emotional Support Animal (ESA) letters
- Other administrative paperwork requested by the patient or family

### Document Processing Fees

Billable paperwork is charged at **$50 per 15-minute increment**.

Time is rounded to the nearest 15-minute increment. For example, paperwork requiring up to 22.5 minutes will be billed as 15 minutes, while paperwork requiring more than 22.5 minutes will be billed as 30 minutes.

**Before the paperwork is completed, you will be informed of the applicable fee. Upon your approval, the charge will either be added to your account balance or charged to the credit card on file.**

### Processing Time

Please allow up to **five business days** for paperwork to be completed. This timeframe allows our providers and staff to review the patient’s information and complete the paperwork accurately and thoroughly.

### Clinical Visit Requirement

A clinical visit is required before paperwork can be completed if the patient has not had an appointment with a MindWeal provider within the previous 30 days.

This requirement ensures that the paperwork reflects the patient’s most current clinical condition and treatment plan.

### Amendments and Corrections

Paperwork will be corrected without an additional charge when a clerical error was made by MindWeal.

Requests to revise, amend, or complete the paperwork again for reasons other than a MindWeal clerical error will be subject to an additional charge according to the fee schedule above.
