- Log in to the MindWeal platform.
- Click "My Profile" as shown in the image below.
- Scroll down to the Account Management section and click "Track Request."
- A window will appear showing the history of your previous account management requests. Click "New Request."
- A new window will appear as shown in the image below. Click "Transfer Ownership", then click "Continue".
- A form will appear, as shown in the image. Select whether you wish to transfer ownership to Patient (only allowed if patient is 18 years and older and is their own guardian), to secondary guardian or to another legal guardian.
- The form looks like shown in the image below. Fill out the form and click "Submit."
- A confirmation message will appear, notifying you that your request has been submitted successfully.
- Once you click "OKAY," you'll be redirected to the Account Management page, where you can track your request's status and view your request history.
TIP:
You can return to this page anytime to track your request's status by navigating to My Profile > Account Management.
- Your request will be reviewed by MindWeal staff. If approved, you will receive an email notification. Additionally, an email will be sent to the new proposed guardian with a link to an online form to accept the role of primary guardian.
- Once the new guardian completes the online form, it will be received by the MindWeal HelpDesk and reviewed by our Managers. The Manager will ensure that the new guardian is prepared to accept the role of primary guardian and its responsibilities before initiating the account transfer.
ATTENTION:
The account transfer cannot proceed until the new guardian completes the online form and accepts ownership of the app, along with the responsibilities of the primary guardian.
