      How to fill out Jotform for Community Outreach Visits

      • Click HERE to open the Jotform titled "Community Outreach Visit Log" 

      • Fill out one field at a time.

      • Fields marked with a red asterisk (*) are required — don’t skip them.

      • Once you submit the form, a “Thank You” screen will appear.

      • To start a new form, tap the refresh button in the lower right corner.

      PRO TIPS: 

      • Open the Jotform before entering the clinic. That way, you can fill in details while they’re still fresh — or even during the visit.
      • If they share a great story or give you permission for a photo, you can take the picture directly through the form.
