For Patients
- Electronic Records (PDF, password-protected, via email): $0
- Electronic Records (provided on a flash drive): $50 for shipping and processing
For Businesses (e.g., Attorneys)
- Electronic Records (PDF, password-protected, via email): $25 (handling charges)
- Electronic Records (provided on a flash drive):
- $0.30 per page for the first 25 pages
- $0.25 per page for pages 26 to 50
- $0.10 per page for each page over 50
- $25 handling charge (includes processing and shipping)
Certification or Notarization of Documents
- Dr. Bansal is the current custodian of all medical records. If a notarized Affidavit of Custodian of Medical Records is required, Dr. Bansal will need to visit a bank for notarization. His hourly rate is $400, and a fee will be charged based on the time spent completing the notarization process.
- Documents can also be notarized in our office by a mobile notary, with the cost covered by the requesting party.
Process
- Authorization: Complete the “Authorization to Release Medical Information” form.
- For patients 18 and older: completed by the patient
- For patients 12 years and younger: completed by a guardian
- For patients 12 to 18 years: completed by both the patient and the guardian
- Payment: The cost of obtaining the records must be paid in advance via a check payable to MindWeal.
Special Notes
- We do not provide paper copies of medical records.
- If the records are requested to support claims for federal veterans' disability benefits, federal Social Security or Supplemental Security Income benefits, or Aid to the Aged, Blind, or Disabled benefits, one complete copy of the patient's records will be provided at no charge.
