Medical Records Release Fees

At MindWeal Health, we are committed to providing secure, efficient, and transparent access to medical records. Please review the information below regarding fees, delivery methods, and requirements for release of records.

MindWeal releases records electronically only. MindWeal does NOT provide paper copies of records, mailed records, flash drives or physical storage devices, In-person pickup of records

For Patients

  • Electronic Records (PDF, password-protected, via email or fax): $0
There is no charge for records requested directly by the patient/guardian for personal use, continuity of care, or transfer of care.

For Businesses (e.g., Attorneys)

  • Electronic Records (PDF, password-protected, via email or fax): $75 (handling and processing fees)

This fee applies to requests made directly to MindWeal by businesses, attorneys, legal offices, record retrieval companies, or other third parties.

Certification or Notarization of Documents

  • $100

Dr. Rahul Bansal is the custodian of records for MindWeal Health. If a notarized Affidavit of Custodian of Medical Records is required, Dr. Bansal may need to coordinate notarization and review of records-related documentation. This fee is in addition to the $75 handling and processing fee.



Process

    1. Authorization: Complete the “Authorization to Release Medical Information” form.
      • For patients 18 and older: completed by the patient
      • For patients 12 years and younger: completed by a guardian
      • For patients 12 to 18 years: completed by both the patient and the guardian
    2. Payment: Applicable fees must be paid in advance before records are released.
      • Checks should be made payable to MindWeal Health and mailed to: 24B Professional Park Drive, Maryville, Illinois 62062.
      • Payments may also be completed online using the secure Stripe payment link: https://buy.stripe.com/dR63dn5yy7a16dy6oo . Under “Patient’s Name,” please enter the firm name or attorney name. In the DOB section, please enter: “Medical Records Release Fees.”

Special Notes

  • MindWeal releases records electronically only. Records may be sent through secure fax or password-protected electronic delivery.
  • MindWeal does not provide paper copies, mailed records, flash drives, or in-person pickup of records.
