Fill Out the Form: Complete the required information and download the card online.

Show It to Your Pharmacist: Your pharmacist should first process the prescription through your insurance, then apply the coupon on top of it to maximize savings. The same instructions have been provided to the pharmacist within the prescription.
Azstarys Savings Card

- Your co-pay should be $0 for the first prescription and $25 to $50 for subsequent prescriptions.
Adzenys Savings Card
