Step 1: Register your account on MindWeal Platform
- Visit MindWeal.com and click Sign Up.
- If you're a new patient (you have never had a clinical visit with MindWeal or BanCAPs), select No
- Enter your child's Date of Birth
- Enter your ZIP code.
- Choose your relationship to the child.
- Select your insurance provider.
- Fill out the Guardian and other required information on the screen. Please specify your preferences to receive notifications and SMS messages from MindWeal. Also review our Terms of Use (link to the terms HERE) & Privacy Policy (link to the policy HERE).
To ensure reliable communication from MindWeal, we suggest using a Gmail address. Specifically, Yahoo.com's email filtering policies sometimes interfere with our message delivery.
- Click Submit. Once submitted, you will see this screen:
- A verification email from MindWeal will be sent to your inbox. The subject will be "Please verify your email." If you don’t see the email in your inbox, check your spam folder.
- Click Verify Email. After your email is successfully verified, following screen will appear, confirming the verification process.
- Sign in to check your dashboard: After creating your profile and logging into the MindWeal platform, you'll be directed to your personalized dashboard. Here, you'll see rest of the onboarding steps presented as tiles. The current step you need to complete will be clearly highlighted for your convenience.
Step 2: Profile Set-up
Fill Out Patient Information:
- Fill out all required details.
- Select your preferred office location from the dropdown menu to receive directions.
- Don’t forget to click Save once you’re done!
Fill out Primary Guardian Details
- The Primary Guardian will be our main contact.
- Phone number and email are pre-filled from your registration. These fields cannot be edited directly. If updates are required, please contact our front desk.
- Indicate if there is a parenting or custody agreement. If applicable, email the agreement to us at contact@mindweal.com. New patient appointments cannot proceed without this document.
- Choose whether the Primary Guardian will also serve as the Emergency Contact. If not, provide separate Emergency Contact details in the subsequent steps.
Fill out Secondary Guardian Details
If there is a Secondary Guardian, this information is mandatory. They will be contacted if the Primary Guardian and Emergency Contact are unavailable. Don’t forget to click Save to ensure the information is stored.
Fill Out Emergency Contact Details
Provide information for someone to contact in case of emergencies when both guardians are unreachable.
Fill out Primary Care Provider Information
Enter your Primary Care Provider’s details for better care coordination.
Enter Pharmacy Information
Choose Genoa Healthcare for streamlined medication refills or enter your preferred pharmacy’s details.
Provide Insurance Details
If we accept your insurance, provide your insurance information, upload front and back side of insurance card, Click Save
Credit Card Details
If we do not accept your insurance, select the Pay Out of Pocket option. Credit card details are not required immediately, but you will have the option to add them later through your dashboard. However, please ensure that credit card details are provided at least 4 days before your scheduled appointment.
Return to Dashboard
- Once you’ve filled out all the required fields, click Go to Dashboard to complete next step of the onboarding process
- The dashboard now features a new tile with a link to self-schedule your appointment. You can use this to conveniently book your new patient appointment.
Important: Please complete the new patient onboarding process at least 4 days before your appointment. If onboarding is not completed in time, our system will automatically cancel your appointment.
Step 3: Sign Treatment Consent Forms
- Once your profile is complete, you will receive an email from MindWeal via DocuSign with a request to review and sign the treatment consent forms.
Check your email inbox to complete this step. If you don’t see the email, please check your spam or junk folder.
Important Note: Consent forms must be signed on a laptop or desktop computer. They cannot currently be signed on a mobile device.
- Open the email and click Review Documents to begin.
- Click if you agree to use electronic records and signature and click continue
- Click Start to initiate the signing process.
- You will need to sign, date, and print your name in each document provided. Click Next to move through the sections.
- When entering your printed name, click Fill In to proceed to the next field. If your name is auto-filled, you must still click Fill In to move forward.
- Once all fields are completed, click Finish to submit your signed documents.
- After completing the process, you will receive a copy of your signed consent forms via email for your records.
- Return to your dashboard on MindWeal app to complete rest of the onboarding process.
Step 4: Complete Medical and Family History
- On your MindWeal dashboard, locate the tile titled Medical & Family History and click Let’s Start.
- You’ll see an introduction explaining the purpose of this document. Click Let’s Go to begin.
Please take your time to provide detailed and accurate medical and family history. Avoid rushing or leaving this task until the last minute, as this information is crucial for understanding your child's medical background. It helps us recommend the most effective and personalized treatment plan. HERE is a sample of a completed Medical & Family History form for reference.
- To submit the document, ensure all fields are filled out, then click Submit to complete the process.
- Return to dashboard to complete rest of the steps of Onboarding process

To begin, click Start Test on the M-CAT tile in your dashboard.
